Care Coordination & Diversion Program


The Care Coordination and Diversion Program has a value base of working in collaboration and partnership with families and providers to improve outcomes for the children referred. Referrals are received from the District of Columbia’s Department of Mental Health. The purpose of the Care Coordination and Diversion program is to provide intensive services to families as a collaborative team in an attempt to build a support system of care for emotionally disturbed children, adolescents and their families. The process is a strength based approach citing that all families have strengths and that can be utilized to mobilize the family in a positive direction.


Our goals are to maintain youth in the community and divert them from placement in Residential Treatment Facilities; in doing so we:

  • Take a Family Driven/Team Approach to treatment
  • Complete a Strengths Needs and Cultural Discovery of the Family (the Family’s Story)
  • We provide individualized and needs based plans of care
  • Facilitate the team approach to develop an outcomes oriented Plan of Care (Treatment Plan)
  • To provide services and support in the most inclusive, normative and least restrictive setting possible (Meetings can be conducted in a variety of safe settings within the community)
  • Provide support, advocacy and assistance to the identified youth and their guardian
